お客さまの個人情報、お寄せいただいた相談内容は厳重に取扱い、プライバシー保護に努めます。

●セルフチェックインのため事前にインターネットより、宿泊者台帳(宿泊者名簿）を宿泊者人数分作成頂いております。下記の項目をご記入の上このフォームでお送りください。

パスポート（日本国籍の方は免許証やマイナンバーカード、学生証などの写真付きの物で大丈夫です）の写真を添付してください。 外国国籍の方は、パスポート、米軍の身分証、在留カードが必要です。
※2人目3人目の必須事項はご予約人数により必須事項がかわります。
※日本の新しいB&Bの法律や所管ルールにより、宿泊者全員の宿泊者台帳(宿泊者名簿）の提出が必要です。

※宿泊人数に相違がある場合は、ご予約人数を変更してください。

※ご予約人数は、自動チェックイン機能により、差異は自動確認できるＡＩシステムになっています。
人数が違う場合、法律に則りご対応させていただきます。
故意による人数が異なった場合には違約金の対象となります。予めご了承ください。
ご予約後、72時間以内にご提出お願いいたします。チェックイン前日の15時までに宿泊者台帳書類のご提出がない場合、入室できなくなります。予めご了承ください。

              個人情報に関して お預かりさせていただきます宿泊者情報及び宿泊者台帳などに関わる本人確認書類・個人情報は漏洩､第三者への提供･売買･共有は所有者の本人許可を得ない限り一切しません｡警察､法律･条令を取り締まる監督管理行政機関からの法的義務を伴う要請が来た場合のみ､お預かりした個人情報などを提出させていただきます｡提出が完了した宿泊者台帳（宿泊者名簿）を送信完了後は、後日、確認ができなくなります。  Regarding personal information We will keep your guest information and identity verification documents and personal information related to the guest ledger, etc. will not be leaked, and will not be provided, sold or shared to a third party unless the owner's permission is obtained.               ※前日までに作成されない場合、チェックイン当日のご案内が停止になります。
              ※チェックイン前日の15時までに宿泊者台帳(宿泊者名簿）を確認できない際は、お部屋へのご案内が出来かねます。当日までお部屋の確保を行っておりますので、その際のご返金のご対応も出来かねますこと予めご了承ください。
              ※宿泊者台帳(宿泊者名簿）のご記入は監督官庁や所轄警察から確認の通達がありご協力をお願いします。
              ※宿泊者台帳の途中保存では宿泊者台帳の記入完成ではありません。完成が終わるまで情報を記入してください。
              ※本人確認書類をデジタルカメラやスマートフォンで撮影いただき、パソコン（スマートフォン）に保存します。 その際、写真や記載内容がはっきりと読み取れる画像をお選びください。本人確認書類については、全体が入るように撮影してください。画像に不備があった場合は再度アップロードをお願いすることになります。

              *We regret to inform you that we may cancel your reservation if you do not fill in the information in the guest register 3 days before your stay.
              * If it is not created by the day before, the guidance on the day of check-in will be suspended.
              *If the guest register cannot be confirmed by 15:00 on the day before check-in, we will not be able to guide you to your room. Please note that we are unable to issue a refund in that case, as we will secure the room until the day of your reservation.
              * Please cooperate with the confirmation notice from the supervisory authority and the police in charge of filling out the guest register.
              *Saving the guest ledger in the middle does not mean that the entry in the guest ledger is complete. Fill in the information until it is complete.
              * Take a photo of your identity verification document with a digital camera or smartphone and save it on your computer (smartphone). At that time, please choose an image that can clearly read the photo and description content. For identification documents, please take a picture so that the whole is included. If there is a problem with the image, you will be asked to upload it again.
